Other players did exist, but currently Ainz is the only player. Every 100 or 200 years, at least 1 player will transmigate into the New World. The reason for this has something to do with the Dragon Lord. As for the specifics, I didn't check

Once you talk to someone for long enough, you exactly are going to know what type of player they are.

Well, players know the world has rules and have some amount of power over certain NPCs depending on how they were programed/designed to serve the player.

Also, players know the unspoken secret that they are in a different world.

Edit: NPCs seem to have freewill to some point, so the only difference is that Players were creators at a point in life and know of the programming.

While NPCs seem to except their design and live how they were designed.

There is a huge difference between regular players and programmers.
The second ones are more influential on the environment.

I would say descendants would have free will.

Only players can tell other people they are players. Remember, players in this world are inhabiting bodies/characters they have created that have a history in that world. So one player + one NPC still = a character with freewill.
NPCs seem to only be the followers that the Players designed through programing, so I would say anyone else in the world (or born into the world) is Not an NPC.

Einz's NPCs were programmed to be following, that isn't necessarily always the case. Foreign NPCs can very much have their own will.

The meaning of programming is; Everything is possible.

Brain sounds different and Hilma's high pitched voice from S1 is just not the same anymore, even that scene where she was supposedly terrified sounded like she was forcing herself. Did they change many of the VAs?
